Mt.Olive 2008 VBS Child Registration form.

      Form Instructions:

1. Complete all boxes. Sender's info must have valid email and phone number with name for registration to be valid.

2. If a box does not apply in the child signup form, you must type N/A in that box.

3. Fill out a form for each child attending. After submitting, just hit the next child button on the Thank You page to signup another child.

    Only hit the submit button once for each child. Make sure info boxes are filled out correctly before submitting. Information will be verified on first night of

    VBS and changes can be made then, if needed.
